Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information.  The notice covers providers and staff at Howard University’s healthcare organizations. This Includes Howard University Hospital, the Faculty Practice Plan (FPP) operated by the College of Medicine, Dental clinics operated by the College of Dentistry, and the School of Communications Speech and Hearing Clinic.

Patient Information

As long as you have not requested that information be withheld, friends and family may obtain your room location and condition. They must provide your name (first and last) for this information. Please tell the nurse if you wish this information to be withheld. Members of the clergy are not restricted from receiving information about you.

Patient Rights

The Howard University Faculty Practice Plan Staff is committed to providing you with the best possible health care. To do so, all Howard University personnel abide by the Patient Bill of Rights.

If you or your family would like assistance understanding the patients’ rights and responsibilities, ensuring that your rights have been respected, or addressing concerns about your care and/or safety, please immediately notify your physician, nurse, the department/unit manager, or contact the HUFPP Patient Experience Hotline:202-865-2150.

If you do not feel comfortable filing a report through this hotline you may email your report to hufpp.grievances@Howard.edu.  All information you provide will be kept confidential.

The Howard University Faculty Practice Plan always strives to provide an exceptional patient care experience that promotes healing and recovery in a compassionate environment. We welcome your feedback, all reports are treated with respect and confidentiality throughout the investigation process, and no one will retaliate against any person making a report in good faith.
