Accepted Health Insurance

Health Insurance

The Howard University Faculty Practice Plan accepts a variety of insurance plans and providers. Take a look at our list of recognized insurers and make an appointment with us today to find out what level of coverage is available to you.

Medicare: 

Howard University Faculty Practice Plan is a participating provider of Medicare. Present your Medicare card, as well as your supplemental insurance cards, upon admission. You will be responsible for any Medicare deductibles, co-payments, and services not covered by Medicare and your supplemental insurance.

 

Commercial Insurance: 

If a commercial carrier insures you, please bring your insurance card with you to the Admissions Office. If your insurance company requires a claim form, please bring a completed form with you. Most commercial insurance carriers require pre-certification, pre-authorization, and/or a second surgical opinion.

 

Medicaid and Medicaid Managed Care:

 If you receive City or have State Welfare or Medicaid Managed Care, please bring your current identification card to the Admissions Office. Pre-authorization is needed for elective admissions or elective outpatient services

 

CareFirst Blue Cross/Blue Shield:

If you are insured by Blue Cross/Blue Shield, please bring your current insurance card with you in order to provide your individual membership identification number to the Admissions Office. All Blue Cross/Blue Shield insurance coverage verification is only an estimate of available coverage and is subject to final review and approval by Blue Cross/Blue Shield. Most Blue Cross/Blue Shield policies require pre-certification and pre-authorization prior to admission. Please be sure to fulfill this requirement before you are admitted to the hospital.

 

Health Maintenance Organizations: 

If you are covered by an HMO, please bring your insurance card to the Admissions Office to provide the hospital with your membership number. Most HMOs require a referral from your primary physician. All HMOs require pre-certification and pre-authorization before admission.

 

Workers' Compensation:

Workers' Compensation cases are covered when approved by the employer or the employer's insurance carrier. In liability cases, you are responsible to the hospital for prompt payment of your bills and are expected to seek reimbursement from the liability insurance carrier.

 

Help for the Uninsured: 

If you don't have health insurance, there may be health insurance programs and assistance available to you through the Affordable Care Act. The Affordable Care Act (ACA) provides options for those seeking health insurance. It provides a health insurance marketplace that allows people to compare health plans, look for a plan that fits their needs, and find out if they may qualify for lower costs. It also sets guidelines for insurance companies, including rules about cost increases, coverage for preexisting conditions, and requirements for certain kinds of coverage, such as preventive care. The ACA works in partnership with individual states. Parts of it may vary from state to state. Be sure to find out the options for health coverage in your state. You can go to www.healthcare.gov or www.hhs.gov/healthcare to learn more about the Affordable Care Act.

Faculty Practice Plan patients have access to The Howard University Hospital cafeteria located on the second-floor. The cafeteria is a full service eatery, serving breakfast, lunch, dinner and a bevy of snacks. The cafeteria is open from 7:00 a.m. to 10:00 a.m., 11:30 a.m. to 3:00 p.m., and 5:00 p.m. to 7:00 p.m., daily. Snacks and drinks can be purchased 24 hours a day from onsite vending machines.

You can count on the Faculty Practice Plan to provide you with the best possible care during the COVID-19 pandemic. Keeping you informed and keeping you safe is our top priority. We operate a COVID-19 vaccine clinic for residents in the local community. Clinic patrons do not need an appointment to obtain a vaccination. However,  you can schedule your vaccine through the  FollowMyHealth® Sign In.

Mail or drop the original form to Howard University Faculty Practice Plan Medical Records Department, 2041 Georgia Ave, NW Suite 6101, Washington, DC 20060. Mailed forms will take approximately 7 business days from the date of receipt to process. We process requests Monday – Friday, records are mailed within 10 business days after processing your request.

Please Note: The Medical Records Department allows up to 30 days to process a request.

Parking

Parking

Parking is available in front of the hospital and outside of the ambulatory care center (Towers Building). The paid parking lots are open from 6:30 a.m.–11:30 p.m. daily.  Upon entering the lots, please remember to keep your parking ticket with you. There is a parking kiosk inside of Howard University Hospital. You will need the tic to pay for parking before you leave.

The Howard University Faculty Practice Plan is committed to providing you with the best possible health care. To do so, all Howard University personnel abide by the Patient Bill of Rights.

If you or your family would like assistance understanding the patients’ rights and responsibilities, ensuring that your rights have been respected, or addressing concerns about your care and/or safety, please immediately notify your physician, nurse, the department/unit manager, or contact the Patient Access and Experience Department via e-mail at hufpp-grievances@howard.edu.

The Department of Patient Financial Services is responsible for assisting patients with matters concerning Howard University Faculty Practice Plan Accounts. Contact Information: Howard University Faculty Practice Plan Patient Financial Services office hours are Monday through Friday from 9:00 a.m. - 5:00 p.m.

Financial Assistance Plans and Programs

Patients who cannot afford to pay for medically necessary care may apply for financial assistance from Howard University Hospital under our Financial Assistance Policy. We encourage patients needing financial assistance to contact the Financial Counseling Office at (202) 865-2130.

Privacy

Patient Privacy - HUFPP

When it comes to your health information, you have certain rights. This section explains your rights and some of our responsibilities to help you. This is a summary of the Notice of Privacy Practice. Read the full version to sign the acknowledgment. I understand that, under the Health Insurance Portability Act of 1996 (HIPAA), I have certain rights to privacy regarding my Individually Identifiable Health Information (IIHI). I understand that this information can and may be used for:

  • Treatment

  • Payment for treatment or services

  • Healthcare operations

  • Appointment/Follow-up calls

  • Coordinating healthcare care among individuals involved in your or your child(ren)’s care

I understand that my or my child’s PHI may be disclosed to outside individuals if:

  • Required by court order, law enforcement, and/or public health organizations

  • Assisting to prevent or control disease, injury, or disability in matters of public health

  • Assisting to avert a serious threat to the health or safety of you, or any other person or the public

  • Assisting funeral directors, medical examiners, or coroners in the event of death

  • Assisting in organ/tissue donations if your child is an organ donor

  • For approved research purposes; patient identification will only be released with your permission

  • Required by federal officials in matters of national security

  • Recommending possible treatment alternatives

  • Complying with Workers’ Compensation cases

I understand that I have the right to:

  • Request a restriction on the use and disclosure of my or my child’s health record

  • Request to inspect or obtain a copy of the child’s health record

  • Request to correct or amend information in the child’s health record

  • Request confidential communications

  • Receive a paper copy of this notice

Howard University Faculty Practice Plan will not receive payment or other compensation from a third party in exchange for using or disclosing the Notice of Privacy Practices. I do not have to sign this authorization in order to receive treatment from Howard University Faculty Practice Plan. I acknowledge that I have the right to refuse to sign this authorization notice.  I have read the full version of the Notice of Privacy Practices. When my information is used or disclosed pursuant to this authorization, it may be subject to re-disclosure by the recipient and may no longer be protected by the federal HIPAA Privacy Rule.  I have the right to revoke this authorization in writing except to the extent that Howard University Faculty Practice Plan has acted in reliance upon this authorization.  My written revocation must be submitted to Howard University Faculty Practice Plan at 2041 Georgia Ave, Patient Grievances Suite 5100, Washington DC 20059.

Bioethics Committee

The hospital’s Bioethics Committee is available to help you or your family deal with issues related to the appropriateness of care and end-of-life decisions. To request a Bioethics consult, you or a member of your family may call the Medical Staff Office at (202) 865-6702.

Pharmacy

Visitors to the Faculty Practice Plans have access to the Walgreens Pharmacy for their medicine prescriptions, which is conveniently located on the first floor of Howard University Hospital.

The Pharmacy hours are:

Monday - Friday 9:00 a.m. - 6:00 p.m.

Saturday: 10:00 a.m. - 5:00 p.m.

Sunday: Closed

Practice Plan Hours Of Operations

If you are experiencing a medical emergency, please call 911 or go to your nearest Emergency Room. The Faculty Practice Plan Clinic Hours are Monday - Friday 8:00 a.m.- 5:00 pm. The Faculty Practice Plan follows the holiday schedule for Howard University.

The Faculty Practice Plan is serviced by the Shaw/Howard University Station on the WMATA Yellow and Green Lines. In addition to four MetroBus routes (70, 79, & 90, 92).
